Addiction Survey for my Adolescent Psychology Class at College?

Hey guys, for my semester poster I am going to do a survey on what people know about addiction and show the results up against the truth.
I need a lot of answers from a variety of backgrounds.
I know it can be a touchy subject, but I would greatly appreciate your help, in fact I NEED your help for my assignment.
Fill in the blank is obvious, but for multiple choice, I will use a, b, c, etc.

Please check the appropriate boxes and fill in the blanks according to your background and

    5. I’d say an addiction is something that you’re so obsessed with that it interferes with your life and keeps you from doing daily activities.

    Yowee. I’m a soph in college and a psych major wanting to work with kids. You get a star and congrats.

    5. Beyond a habit it may also be a compulsion, often driven by the substance and it’s effects, as well as external influences suggesting one cannot deal with life, without continuing to explore the addiction.

    6. It can be thought of as a subjective word, and have variables. IE: I seem to be addicted to shoes, though it’s never truly affected me adversely.

    7. Some are more deeply rooted, even if not enjoyed. CURE is probably as subjective a word as Addiction. IE: I know people who quit smoking for as long as 20 years and went back to it.

    8. How is often situational, circumstantial, and again influenced.

    10. That ties into good or bad, and the list could be volumes here. Obvious ones are any substances. Any suggestions of sexual addictions, or mine a shoe "addictions" don’t really fall into the the classification in a strict sense.

    11. Another I can’t personally attest to. I love shoes.
